Wow,
IRPCS (and Inland Rule 14) about head on situations requires both vessels to turn to starboard. i.e. To pass Red to Red. It says nothing about passing 'Green to Green'.

But USCG AtoN book page 5 shows Green to Green as 'Acceptable'.

That must cause everybody a moment of doubt in every head on situation whilst each waits to see in which direction the other vessel will turn.

This was the reason why green to green was not included in the COLREGS.
Sound signals cannot be relied upon at sea.
