the spouse has been on warfarin (aka rat poison) for 10 years. we don't voluntarily sail in more than 20 kts. as it bangs the crew around too much. we carry a 'trauma pack ' on the boat which is a first aid kit wilderness medicine add on for a severe cut or laceration that pressure and ice won't stop. no more bosun chair work, no climbing the mast steps, no rail meat work and no racing. we sail the coasts and are always within call range of the USCG medi evac helicopter. i got both my EMT license and a ham license.
(alas, we now have abandoned plans to sail the atlantic to england, or to cuba....) but at least we are still sailing ! (with combined years over 146..)
