at 800 433-7245 or 724 763-2229. They could probably fax you the form or just take your information over the phone. They have my jib right now and they are aware that their web page is down.

Just an aside... I got my sail back from SailCare and it looks GREAT! Haven't hoisted it yet - too cold
But, needless to say, I can't wait for Spring!
