Actually it can be done with the boat in the water. I once put a new
thru-hull in with the boat in the water. Took a diver and a gadget (like a
bowl with a foam edging) to go over the hole drilled from the inside. Then
once hole is drilled and guy inside blocks the hole (with something like
neoprene) and the diver puts the thru-hull right in and holds it in while
the nut is threaded and the ball valve was threaded on. By the way, we also
removed a packing gland and installed a dripless shaft bearing with the boat
in the water also. That was pretty easy. In neither case did we take on more
than a liter or two of water.
I wouldn't recommend that it "be tried at home" though.
