Wow, that quite a story.
Having just done a re-alignment I am amazed that the adjusting nuts could "back off". I suppose anything is possible, but with the initial tightness of the nuts and the paint/slight corrosion on the threads it seems unlikely.
However, I won't second guess someone with eyes on the situation, and I can't come up with another senerio.
Was there any damage to the strut itself? If there was enough force to break the cutlass, I would be looking at the strut very carefully.
