www.islandnautical.com is the only "mail order" dodger company I've seen... you still need to do some measuring.. they will have a form. Even these ready made models require fitting on your part.. which makes a custom made one actually seem reasonable. Dodger's are expensive... that's why so many people opt for the bimini which you can easily order from any chandlery and assemble yourself.
