Changing documented name

Searching the Coast Guard web site and forums are turning up incomplete data. My vessel is already documented and I HATE the name. ( No, I won't tell you what it is because there are alot of them out there and I fear retribution.)What are the proper CG forms to fill out? A google search turned up the CG 1258 but does not look correct. Anyone? Thanks.

Various Forms

There are several forms that have to be filled out "if" you have a loan on the boat... if I get a chance I will do a search on the web - there is a documentation site under coast guard that gives the directions.... if no loan it is easy and one form... just don't recall the #

It appears that is the correct form,

This is from the Vessel Document Center Website http://www.uscg.mil/hq/g-m/vdoc/nvdc.htm FAQ page which states: HOW DO I CHANGE THE NAME OR HAILING PORT OF MY VESSEL? The name and/or hailing port may be changed by filing an application for change on form CG-1258 with the appropriate fees. If your vessel is subject to a mortgage of record, you must obtain permission from the mortgagee on form CG-4593. I thought it was $50 for a name change, but fees indicate $84. I called them once about an address change and service was very good, call if you have questions.
