It appears that the HOH in my bilge is a result of a reversal through the bilge pump when underway and the through hull at the transom is under HOH. What would be the best way to prevent this?

Best way to prevent siphoning of water back into bilge

Most bilges on sailboats drain out of the back of the transom above the water line. I have also seen vented loops in them.
Hard to tell how much HoH there is because the bilge pump goes on, but there is at least an inch that just about covers the Keel Bolts

The check valve and vented loop suggestions will work, but the very best solution is to move the discharge outlet. If yours is above the waterline when docked, a pretty simple project.

Check valves in bilge pump systems are not recommended. Theoretically it is a great place for their use. In reality, check valves get clogged. Think worst case and guess when that'll happen. The only place I have a check valve on our boat is between the shower sump and the shower discharge pump. Put in a high enough loop and it'll stop water backfilling.
