Bedding Teak rails w/ Butyl Tape and Screws?

Removed some teak grab rails and toe rails and planning on the reinstall. Read MaineSails thread on bedding hardware but still have a couple questions.

1. Do I use the butyl tape under the entire teak toe rail or just at the screw holes?
2. The grab rails are screwed in from under the deck into the grab rail. Can I coat the inside of the screw hole on the grab rail with a bit of epoxy to close up the hole a bit so the screw grabs better? Should I use something else instead of epoxy?
3. Same question, different material. The toe rail is held on by screws going through the rail into the fiberglass deck. Can I close up the screw holes in the deck with a coating of epoxy so the screws will grab tighter? Will use a syringe to get the epoxy in the hole.

One last question. Why does every screw and bolt on my boat have slotted heads instead of phillips heads? I hate slotted heads! I know my boat is old but they did have phillips head bolts and screws 36 years ago didn't they?
