Clear Lake woman dies after falling 35 feet off mast of historic tall ship ELISSA in Galveston

The 58-year-old was a volunteer on the ship at Pier 21, according to the Port of Galveston police chief. Officials say she was wearing a safety harness at the time.

That's rather unclear, but I would guess that although she was apparently wearing a harness, it may not have been attached at the moment of the fall, or not attached appropriately ..
