Yanmar SD50 recurring seal issue

Hi Guys I will be hauling out my catamaran to fix a water in the port saildrive oil problem. I was hoping that someone could help with the possible causes of why the SD50 reseal leaked again. Here's the history: The drives have just under 4000 hours on them. The problem was first noticed in April by the surveyor I have no history before that. The drives were resealed but not by me and I was not there for it and I have no information other than they replaced two seals and two o-rings. I believe that about 50 to 60 motoring hours were logged and now the port drive oil looks like a chocolate milkshake again. I'm just trying to anticipate what to expect once hauled.
1. Fishing line wrapped and damaged the new seal
2. Shaft wear that should have been detected in April (possibly need speedy sleeves installed)?
3. Excessive bearing play that should have been detected in April
Any comments on what is the likely cause? What other things should be checked? Best mechanic or yard in the middle to upper Chesapeake?
Any help you guys can offer will be greatly appreciated.

Covered pretty well. Only other is a bad o-ring seal. Another might be a pinhole in the casting

Thanks kloudie1 I hadn't thought to look for that.. I'm not sure the condition of the housing but I'll inspect carefully.

Good luck ..
Just a smidge of corrosion/debris left in the o-ring seal groove might be something to look for when the new seal is being worked.
