Your extender unit should have a hitch ball about two feet from the end and a receiver hitch at the front. The end is kept from swiveling by U-Bolts or through bolt or pin to the trailer.

Patrick; You probably won't be able to buy one,but you can have one made. Take your trailer to a trailer repair shop, or maybe even a muffler shop. Tell them what you want, with a simple sketch. They'll be able to fab up something that will work for you.
