I recently renamed my CG documented sailboat. Can anyone tell me the form (and form number) and fee that I must submit to the coast guard to apply this change.

Call them. They are very helpful. Make sure you call them today though. They probably will not answer come Monday.

It is posted on the documentation website for you to print out along with a check. They are a very good operation to deal with. Especially considering that it is a federal government agency.

"HOW DO I CHANGE THE NAME OR HAILING PORT OF MY VESSEL?
The name and/or hailing port may be changed by filing an application for change on form CG-1258 with the appropriate fees. If your vessel is subject to a mortgage of record, you must obtain permission from the mortgagee on form CG-4593."

Do NOT operate the vessel until you have the documentation changed! Your documentation becomes invalid as soon as you alter the marking (Name and Hailing Port). Fines are steep, I think about $10,000 a day although the probability of their actually being assessed is low. Still, you could face a mountain of paperwork if the UCSG boarded you.

If the government does shut down, I suggest you put the old name back on before untying the docklines.
