Is realigning the engine/transmission something that needs to be done on a regular (yearly or every other year) basis?

Nope

Alignment should be checked when the boat is hauled and launched, or when there is work on a part of the drive system. Other than that, from time to time as the motor mounts settle (years between).

Hope not!

That old 2QM20 in my 1979 H37C has not been aligned since I have owned the boat. We motored out to sail yesterday starting our fifth season. I do check the mounts to make sure they are snug and that the motor is not moving around on them. And I watch the shaft the first couple of times while motoring out the river, more to check the packing but watching for any "wobble". While hanging in the sling before the big splash the shaft was as snug as ever in the cutlass bearing. If it had been out of alignment I would expect the bearing to go. Are we just lucky?
