I'm filling out the form for the first time to get the PHRF rating for my 1985 Catalina 30, standard rig. Does anyone know the HB, MGU, and MGM measurement for the stock mainsail?

There really is no such thing as a standard sail. Each are made differently unless they are strict one design sails. And then THEN you still measure.
PHRF will want to make sure your dimensions are less than the max allowed. That is a function of your foot or 'E' size.
MH (headboard) <= 0.04*E
MGT (7/8 leech) <= 0.22*E
MGU (3/4 leech) <= 0.38*E
MGM (1/2 leech) <= 0.65*E
MGL (1/4 leech) <= 0.90*E
