I think about a number anchorages which are otherwise close to entrances. I guess experienced "professional" pilots who go in- and out- of such entrances might not be so reliable.

The captain of the Steamship Authority ferry Iyanough mistook a metal pole and two moored sailboats as buoys leading into Hyannis Harbor before the vessel hit a breakwater.
