Welcome to the forum and congratulations on your new sailboat!!

Handrails: make measurement based on when the holes were or from another 25.5 or buy a 3 loop Whitecap teak handrail for $38 as shown in the 25.5 photo below that fits from this site: Whitecap Industries Whitecap Teak Handrails (sailboatowners.com) or from Amzon or on-line or local marine chandlery like West Marine and include screw hole plugs.

Companionway top sliding teak: Since it is custom you will need to fabricate yourself from teak, marine plywood glassed over OR starboard or acrylic sheet of the desired thickness cut to fit by your local plastics store. I replaced my slider rails with teak "bunk boards" which are available from on-line teak sellers.
Based on current teak prices, they are not cheap .
I think that's what Hunter used on my 79 H30C originally and it works fine.
