I would like to add OB with reverse and external fuel tank, but have to go to a 4HP or larger to get these options, new 4 strokes weigh about 58 LBS. Is that too heavy?

Absolutely Not!
I once had a C25 w/8 hp Sailor (Evinrude) on it. It had an external tank - no problems. Even a smaller C22 or an H23 will be OK. Just realized you are speaking of an 18.5-footer. Check with local shops about the weight of the Sailor.
