My macerator pump runs, the blades spin, and there is no obstructions in the lines, yet does not discharge overboard. Does the holding tank need to be completely full, or do I need to replace the pump? Thanks for any insights

The pump sounds normal, but does not get that high pitch whine when the tank is empty. I checked everything except the vent, Thanks. (and yes, the thru hull was not overlooked)
They fail. What pump do you have? Jabsco have a terrible reputation, 'cuz the guts literally rot out. I just had my 27 year old Jabsco replaced with the Shurflo. They look the same and fit in the same space, but I did a LOT of research and homework to choose the replacement manufacturer. Johnson has a good reputation also.
Given you situation, why not simply fill the holding tank with fresh water and see what happens?
You have nothing to lose, and get to clean out the tank.

Good point, just an real world opportunity to say "They don't make 'em like they used to!" 
When researching a replacement, I noted many horrible reviews of Jabsco macerator pumps, not so much for the Shurflos and then learned about the differences in the way the guts are made. One of the reasons many of us keep suggesting people do their research and homework.
And, like mot other mechanical pieces of gear, if they don't get used they seize up. I am fortunate to be able to sail weekly and often go out into the ocean.
