No cotter pin?, No second jam nut or a small screw in an acorn nut that puts back pressure on the tip of the prop shaft?
Depends upon how the prop is secured. My Flex-O-Fold does not have a locknut nor a cotter pin. The nut is recessed and locked in place with set screws from the sides of the hub. If the key were to shear the prop could unscrew the nut with enough hard reverses.
It has and it did, in the Erie Canal. I replaced the bronze key with a stainless key. I have a shaft-saver and do not need a "shear key". I got the prop back in November when they drain the canal for maintenance.

Yes I also have the flex 0 Fold on my H-36 and when I heard what happen to you I did worry for a wile but don't think about it any more and just check it when ever it's out of the water,no locking nut or cotter pin like the stock prop.
It works great and love it but you do need to keep the prop clean,when I went up to NY for 3 month's I came back to Florida I said to wife let's go sailing and as I pulled out of my slip it would not move even after I floored it would not move hardly so I quick got back into my slip and next day dove on it and it was loaded and would not open,after a hour or so of diving under and scraping and cleaning it worked just fine.
They just took the staples out of my back today and I am feeling pretty good and will see the surgen thursday and as soon as he gives the OK will will dive to clean the prop and go sailing.

So you are no longer stapled together Nick? Hope you don't come apart. At least wait until the glue dries before you go in the water. Do you have a suit? I pay a diver monthly. Not sure I would want to swim in the marina. He always reports how bad the shaft and prop are. I would think there is enough copper in that alloy that we would not be bothered so much. Your story makes me hope the diver is doing a good job. As you know one of the things about the Flex-O-Fold is that a zinc covers the end of the hub. One reason for the recessed nut. In this picture the zinc was not yet installed.

in fixed prop , the prop blades are mounted on rubber bushing
they may slide off it.
only the blades separated from the central part / spline
the reasone is that the fixed prop was fixed to place by a small diameter nut (supplied by yanmar for foldings prop ) which is wrong
fixed props should be fixed by large diameter cone nut which keeps in place central spline part and also rubber part with blades
i have used the small nut since yanmar manual states that the cone nut is optional which is not clear and misslead
