If you qualify for Medicare then Blue Cross Blue Shield Care First supplemental would probably get you care in most places and cash is always accepted everywhere.
Take a look at DAN (Divers Alert Network) ... this is 'trip/med' insurance and covers evacuation, on location med (up to US$10K) expenses, etc. This is through the infamous AIG group so you can probably expect to see decreased benefits in the near term. You dont have to be a 'diver' to subscribe

I would look around for some basic coverage with a $10k deductible. Once you leave the USA the cost of medical costs may drop to a cost where you can pay our of your pocket. If you are planning on going to Mx. most cost are reasonable. I do not know what you are expecting, but the most important aspect is the ability to be medivaced back to the USA if somethign is beyond the capability of these foreign doctors. If you need expensive medications, you may be stuck with a couple of policies to cover everything that you may need.

Just Went Through This a Year Ago
A bunch of friends went went on a boat trip. Different groups on different legs some all the way to Grenada. What we found was the best deals were from your Medical Insurance carrier or from whoever your broker is for home boat auto etc. Some will guarantee medical evacuation to the states some to the closest point of acceptable care, your call on that one.

Finding health care insurance to cover you anywhere is going to be expensive and the problem will not be if you have insurance, the problem will be, no one will take it. Anyone with the means to buy better does not visit the free services in the Carib/Central/South America. However, the fee-based services are VERY cheap compared to the US and usually of VERY high quality. Not ALL of them, of course, just like not ALL US doctors are high quality. We have found medical care , at a for-fee service, in Central and South America to be excellent.
You can receive excellent dental care , including some significant work, for peanuts, compared to the US, from a US-trained dentist, at excellent prices. The same care in the US would have been 10x in cost, and unlikely to be better quality. Also had a small surgical procedure done in the US that cost more than 10x times what it would have been in Central America, with NO better quality, and MANY unnecessary procedures. IMHO, the medical system in the US is fundamentally broken and unlikely to be fixed in our lifetime.
You can't get good care on island with a population of 10,000 or less no matter what insurance or health plan you have. Just like you can't get excellent care in Oxford Maryland (population 1000), or any other US city of less than 5000 people. But if you go to Barbados, Trinidad, Venezuela, Panama, Cartegena, or similar large-population center, and then get references, just like you would to find a doctor in the US, you can get Excellent care at bargain-basement prices.
