FCC 605 and 159

Hello!
I am trying to obtain an FCC Ship Radio Station LIcense. Has anyone done this?

I have a Hunter 49.

On the item 5 of the 605 form should I use VLR for sailing ship or YAT for Yacht?

Use YAT. Don't worry too much about accuracy. Remember, these forms are designed primarily for commercial craft.

Thanks Rick, in the form 159 to submit payment what's LOCKBOX #?
I have not been able to find the po box # to fill this item.

Miguel, looking through my old records, I find:

Wireless Bureau Applications
PO Box 358130
Pittsburgh, PA 15251-5130

However, if you can do this on-line rather than by mail, I strongly urge you to do so. It is far less confusing and prompts you as you navigate the forms.
