Companionway door
There is a picture of such a door in the Sept/Oct 2002 issue of "Small Craft Advisor on page 22. It is on a boat called "Sea Biscit". Looks like he just started with a single piece of wood, cut it vertically in the center, hinged it on each side and put a 6" clear pop in-and-out deckplate for a port hole which is convenient.
