You have to work from both the inside and outside for both 'sets' of chainplates.
The lowers are the easiest to replace, the uppers require a more extensive removal phase, and may require help from someone else.

This is a link for newer reinforced lower chainplates. The uppers are fastened to the bulkheads and the replacements seem to look just like the originals to me.
Of course I see that you have a Hunter which makes my link kind of foolish!!!
