Bow eye replacement on a 22

Does anyone have advice on replacing the bow eye on an older (1974) O'Day 22? The original has fallen out, but I cannot access the old bolt as it is either burried under a bunch of epoxy or foam floatation in the bow. Does anyone have advise on how best to attack this problem and how to ensure the new one doesn't pop out?

Hard to visualize

I am having a hard time understanding what happened. On my '73 22 the bow eye is above the area of where the foam is. I can access it at the "anchor locker" (just the furthest forward area of the cabin where I store my rode. I guess that I am having a hard time understanding because my bow eye and the corresponding bolt are one and the same, thus, unless the bolt broke, the bow eye and the bolt would come out together. This would occur only if the nut and backing plate failed. When replacing, I would try to find a bow eye like this and make sure to have an appropriate backing plate, washers, and nylock nut.
