oDay 22 roller furling hardware?

My 1980 O'Day 22 has a roller furling, and we were notified by the marina that something had gone awry; it had somehow come undone while docked. It appears that the hardware piece that connects the halyard pulley to the mast had failed. What is the best way to repair this and what hardware is needed? Thanks for the help!

Here is a pic of the halyard block for the headsail on my 1980 O-Day 22. The tang above it is for the forestay. Perhaps the clevis pin came out or maybe the swivel block has failed.

Thanks a lot, that's exactly what I needed. Couldn't remember what was there once it was gone! That's a big help, should be sailing with Jenny up real soon.
